Reinstatements are based on the time since a license expired:

• Within one year of the license's expiration; or
• More than one year after the license's expiration.
• Reinstatement of restricted license

Reinstating when License Is Inactive For Up to One Year

An individual whose license has been inactive for up to one year and who wishes to reinstate to active status must do the following:

• Submit an application for reinstatement to the Board;
• Provide documentation of completion of category 1 continuing medical education (CME) and mandatory training on identifying and reporting abuse, if applicable; and
• Pay the reinstatement fee.

This application is not available on the website.  Call 515 281-5499 to have an application sent to you.  All required information and payment must be received in the Board office within one year of the license becoming inactive to qualify for reinstatement in this manner.  For example, a physician whose license expired on March 1 has until the last day of the following February to reinstate. 

The amount of CME needed to reinstate is the same amount that had been required on the most recent license renewal.  No hours earned in the inactive period may be carried over to a future license period.  Mandatory training on identifying and reporting child abuse or dependent adult abuse is required every five years of physicians who provide primary care to adults and/or children.  Click here to read about mandatory training.

Upon receiving the completed application, staff can administratively reinstate the license to expire on the renewal date that would have been in effect if the licensee had renewed the license before the license expired.

The applicant who fails to submit all reinstatement information required within 365 days of the license expiring shall be required to meet the reinstatement requirements for licensees who have been inactive for more than twelve months.

Reinstating After License Is Inactive Longer Than 1 Year

An individual whose license has been inactive for more than one year must do the following: 

• Submit an application for reinstatement to the Board;
• Pay the non-refundable fee, which includes a criminal history background check.
• Provide documentation of completion of 80 hours of Category 1 CME within the previous two years;
• Provide documentation of completion of mandatory training on identifying and reporting abuse, if applicable;
• Verify other state licenses, hospital privileges, and any new post-graduate training since the licensee was originally licensed;
• If the physician has not engaged in active practice in the past three years in any jurisdiction of the United States or Canada, the Board may require the applicant to:
• Successfully pass a competency evaluation approved by the Board;
• Successfully pass SPEX, COMVEX-USA, or another examination approved by the Board; or
• Successfully complete a retraining program arranged by the physician and approved in advance by the Board
• An individual who is able to submit a letter from the Board with different reinstatement or reactivation criteria is eligible for reinstatement based on those criteria.
