Reinstating when License Is Inactive For Up to One Year An individual whose license has been inactive for up to one year and who wishes to reinstate to active status must do the following:
This application is not available on the website. Call 515 281-5499 to have an application sent to you. All required information and payment must be received in the Board office within one year of the license becoming inactive to qualify for reinstatement in this manner. For example, a physician whose license expired on March 1 has until the last day of the following February to reinstate. The amount of CME needed to reinstate is the same amount that had been required on the most recent license renewal. No hours earned in the inactive period may be carried over to a future license period. Mandatory training on identifying and reporting child abuse or dependent adult abuse is required every five years of physicians who provide primary care to adults and/or children. Upon receiving the completed application, staff can administratively reinstate the license to expire on the renewal date that would have been in effect if the licensee had renewed the license before the license expired. The applicant who fails to submit all reinstatement information required within 365 days of the license expiring shall be required to meet the reinstatement requirements for licensees who have been inactive for more than twelve months.
